Survey Background

Envision Solutions, LLC and Trusted.MD Network are conducting the 2007 Taking the Pulse of the Healthcare Blogosphere survey to collect important demographic and opinion data from the global healthcare blogging community.

This survey is open to all self-identified bloggers who devote at least 30 percent (30%) of their posts to the topic of healthcare.  The survey will close on October 15, 2007.

Information Collected (Survey)

By taking part in this survey, you agree to provide Envision Solutions and Trusted.MD with demographic data and information on your blogging habits, preferences and opinions.  We will also ask you to opt-in to provide us with:

- Your e-mail addres

- The name and URL of your blog(s)

Envision Solutions and Trusted.MD have contracted with Survey Monkey, a leading online polling company, to administer this survey.  Survey Monkey will place a “cookie” on your browser, so that you can resume taking the survey at a later date.

Cookies are small text files stored on your computer by a Website that assigns a numerical user ID and stores certain information about your online browsing.  The cookie file does not store your personal information.  If you prefer not to receive cookies from Survey Monkey, you can disable their use in your browser settings.  By doing so you may not be able to fully participate in this survey. 

We will not collect personally identifiable information as part of the survey.  However, if you are selected to receive a survey incentive, we will contact you to provide us with your name and contact information for the purposes of providing you with your prize.  Your personal information will not be shared with any third party or revealed to the general public without your express permission. 

How Survey Information Is Used

Envision Solutions and Trusted.MD will use the information you provide us in the survey to:

- Gather, aggregate and analyze survey data

- Provide the results of our research (which contains no personally identifiable information) to media and potential clients (communications firms, non-profit organizations, government agencies, marketers and others) and other interested parties

If you agree to provide us with your e-mail address, we will use it for one or more of the following purposes:

- Contacting you with information about this survey

- Contacting you to participate in future surveys on the healthcare blogosphere

- Entering you into a drawing to potentially receive survey incentives

We will provide you with the opportunity to opt-out from future mailings in all of our communications. 

Sharing Information

Your survey responses and e-mail address (if provided in the survey) will be shared between Envision Solutions and Trusted.MD for the purposes of analyzing data and awarding survey incentives.  Survey Monkey will store, but not access to your survey responses. 

Envision Solutions and Trusted.MD will not:

- Share your e-mail address or any other personally identifiable information with third parties

- Sell your e-mail address or any other personally identifiable information to third parties.

Envision Solutions and Trusted.MD will make every effort to protect your privacy.  However, if required to comply with a court order, legal process or other lawful purpose we may need to disclose your personal information.
